Make a secure payment using your Kareo/Stripe portal account you previously created with the email address you provided our office. You can use this link if you have paid from our email invoice in the past. You can pay by credit card, debit card, or from your HSA account.
If you do NOT have a Kareo/Stripe portal account and have never paid from our email invoice in the past, you can still make a secure payment on your account with a debit or credit card. (Powered by PayPal). A PayPal account is NOT required to use this payment feature.
MasterCard/Visa/Discover Credit Card/Debit Cards and Health Savings Account Cards Accepted at the point of care.
Access the records for which you have requested access in the past here. You will need to have your USERNAME (usually the original email address you provided) and the password you have chosen (password can be reset).

Please check with your insurance and our office to confirm that we accept your insurance and are in-network.
A $50 No-Show Fee deposit is required to make a first-time appointment. This deposit is refundable if the new patient keeps their first scheduled appointment or cancels with notice of 24 hours or more, otherwise the deposit is forfeited.
New patient paperwork must be signed and returned to the office before a first-time appointment can be scheduled. Please allow us at least 48 hours to process the paperwork. See the downloadable/printable forms below for the required paperwork. Signed forms can be returned by mail, in person, fax 949-437-2016 or email (firstname.lastname@example.org).
Once all the paperwork and deposit are received and the information is verified by staff, an appointment can be made. We cannot promise same-day appointments for new patients. Please note that there may be additional paperwork to fill out at the first visit. Please arrive 10-15 minutes early.
